Currently, the Medusa Scythe from the PG is bugged in a bad way.

Also, some form of equipment upgrade has a bug, wherein the +9 equipment shows "no indication of contamination" despite the fact that it always has if you try to upgrade to +10. It is somehow verified, and SEGA is currently doing something about it.
